چکیده

A 32 year old woman presented to a hospital in North America in November 1996 with upper abdominal pain and pyrexia. Liver function tests and ultra-sound scan of the abdomen were normal. Gastroscopy demonstrated an abnormal opening on the lesser curve of the stomach which on barium meal investigation appeared as a sinus tract extending superiorly (fig 1A). A computed tomography (CT) scan performed showed minimal aerobilia in the left liver but no other abnormality. At laparos-copy, a well formed tract was identified running from the gastric lesser curve towards the left side of the hilum of the liver. This was excised using a linear stapler. Histology revealed biliary type epi-thelium in the excised segment of the tract. She presented to this unit in August 1997 with further episodes of abdominal pain and fever. A CT scan was performed and this showed mild dilatation of the left intrahepatic ducts. Endoscopic retro-grade cholangiopancreatography (ERCP) demonstrated stenosis of the left hepatic duct with a large proximal concretion (fig 1B). In addition, there was a diverticulum passing inferiorly from above the stenosis which corresponded to the upper end of the previously divided tract. Laparotomy and left hepatectomy were performed. The re-sected specimen revealed stenosis of the left hepatic duct 1 cm above the hepatic duct confluence. The left intrahepatic ducts were grossly distended and filled with sludge distal to the stenosis. This patient made an uneventful recovery and remains well four years since surgery. DISCUSSION The incidence of aberrant anatomy of bile ducts varies from 16 to 31%. 1 2 Aberrant or accessory hepatic ducts from the right side of the liver are more common with drainage into the right hepatic duct, cystic duct, and occasionally into the gall bladder. There have been isolated reports of separate entry of the right and left hepatic ducts into the duodenum. There are no reported cases in the literature showing drainage of a bile duct into the lesser curve of the stomach. 3 4 The presence of an aberrant duct in the free edge of left triangular ligament has also been reported. 5 Distal communication of this structure with the gastro-intestinal tract is not recorded and this anomaly probably represents distal extension of atrophic hepatic elements into the left triangular ligament rather than a true functioning duct. In the present case, the structure divided laparoscopically was most likely an accessory left hepatic duct. As there was …

Double Common Bile Duct with Ectopic Drainage into the Stomach Found in Asymptomatic.

The case of a patient with asymptomatic double common bile duct that was identified by chance is presented. A 41-year-old man underwent esophagogastroduodenoscopy(EGD) as part of a regular health checkup, during which he was found to have an elevated lesion in the lesser curvature of the upper gastric corpus with bile draining from its tip. Further examination led to a diagnosis of double commo...

Sonogram of Biliary Dilatation in Children

The biliary trees descend from the canaliculi at the hepatocytes in the liver. Bile drains from this level into intrahepatic ducts, smaller ducts combine to form the segmental bile ducts which within the liver gradually enlarge and merge to the right and left hepatic ducts. The right and left hepatic ducts join together to form the common hepatic duct (CHD) at porta hepatis. The common bile duc...

Six rare biliary tract anatomic variations: implications for liver surgery.

OBJECTIVE The variations in the anatomy of the biliary tract need to be recognized in modern liver surgery. The purpose of this clinical and anatomical study is to describe several novel biliary tract variations and to outline their practical importance for liver resections and transplantations. MATERIALS AND METHODS Over the previous 10 years, the anatomic variations of the bile ducts were e...

Double gallbladder completely enclosed in a cystogastric fold of peritoneum

Double gallbladder is one of the rare congenital anomalies of the gallbladder. Failure to detect an accessory gallbladder hampers diagnosis and treatment of cholecystitis, which might result in recurrent attacks of cholecystitis. In addition, presence of peritoneal folds extending from the stomach and duodenum to the gallbladder is very rare. Here we report the presence of a double gallbladder ...

Common and Uncommon Anatomical Variants of Intrahepatic Bile Ducts in Magnetic Resonance Cholangiopancreatography and its Clinical Implication

BACKGROUND Preoperative knowledge of intrahepatic bile duct (IHD) anatomy is critical for planning liver resections, liver transplantations and complex biliary reconstructive surgery. The purpose of our study was to demonstrate the imaging features of various anatomical variants of IHD using magnetic resonance cholangio-pancreatography (MRCP) and their prevalence in our population. MATERIAL/M...
